HHA/STNA – ALTIMATE CARE

Altimate Care is hiring compassionate and responsible Home Health Aides (HHA). The HHA will provide assistance with daily living tasks and personal care services to clients in their homes in accordance with an established care plan.

Altimate Care provides home health services to our patients by delivering one on one nursing care in our patients’ homes. This helps promote healing, reduces stress and allows the client to remain in their home. We’re seeking a HHA/STNA to join our team!

Responsibilities

• Provides help with personal tasks and personal care services in clients’ homes
• Completes appropriate visit records timely and accurately
• Acts as a liaison between the patient and care team.
• Communicates changes in the patient’s condition and/or needs to the care team
• Health promotion and maintenance

Qualifications

• Ability to participate in physical activity, including but not limited to standing and lifting
• Ability to perform job for extended periods of time
• Organized and dependable
• Able to work independently
• High school degree or equivalent required
• Must have listening/verbal communication skills
• 1-year experience of home health experience in the past 2 years under the supervision of RN, OR current STNA licensure
• CPR/first aid certification
• Valid driver’s license

Schedule

Monday to Friday 8:30am to 5pm
